Orchid leaves turning yellow-brown color

My orchid had borne flowers twice before its leaves became yellow with round-shaped brown color in the middle where withering started. Then the part of the leaf attached to the stem was obviously rotten and eventually the plant died. This has been my second time losing an orchid plant for the same symptoms. What should I have done to save those poor orchids? 🙄
